Software Documentation Crisis

How does internal software documentation kept in your teams? Who is responsible for updating it?

I've started in the new company and it takes me enormous amount of time to get myself up to speed. I have thousands of files to read with documentation but none of it reflects the current structure since it's outdated. Wondering how does it work for other teams? What is the most efficient on boarding process for new dev?
