How do you fill in a revision history table for the first time in the IEEE srs template?

My professor didn't explain this and I'm not sure how to correctly fill in the revision history table that is below the index.

The columns are name|date|Reason for change|version

I googled revision history of other software to see how it's done but it was different.

I usually put Reason for change as initial document creation, version as 1.0.0, and date as the date I created the srs. I'm not sure about name though. Do I write my name or the name of the software, or does each version of the software get a new name that is different from the software's name?
