Custom Software Discovery / Requirements Gathering Fillable Questionnaire for Non-Techies, Does it Exist?

Hello everyone. Has anyone created an excel sheet, or even better, something like a questionnaire (Google Forms?) that can be sent to non-technical clients to begin the Software Discovery process, be it for an MVP or a full software system?

There are always the same questions asked with each software discovery when it's first starting off before it starts to get specific. To send a client an introductory questionnaire seems like a no-brainer. Thank you!
