Documenting stored procedure changes and code changes

Fellow redditors, I come to you seeking help from the wise ones!

In my company, we're developing some software that is being used in different countries. Each country has it's specific exceptions that are used only in their country. A simple example: we have a different set of national holidays than they do. To make things even more complicated, we regularly update the version in our main country. Other countries get the latest version once a year!

Which brings me to my problem. I do not remember which stored procedures were changed in the last 12 months.

I'm looking for a way to document changes that were made in the last months. That way I can just update the database with the new additions. So far, I'm using a simple word document, to write stuff that I've changed. I'm positive that this is not the best way.

What do you guys use? Asana, Jira?
