How often are best practices and design patterns actually utilized?

I've worked at a few different organizations in my lifetime, and I usually only see an focus on new tooling, frameworks, etc and less of an emphasis on coding best practices and utilizing appropriate design patterns.

I've also interviewed a ton of developers, and the overwhelming majority of them were not familiar with common design patterns such as factory and dependency injection.

Is anyone else seeing the same thing?
