Looking for a program recommendation

Hi everyone, I have been looking for a form making tool to integrate with my program and I have been having trouble finding one that does what I want, and was wondering if anyone had any recommendations. Here is my use case:

The form needs to be open with an option to add a new card for the user. When he is making a new card, he can check a bunch of options for that card. He can make as many cards as he needs and then he can submit the form. Basically I need a form tool that has an undetermined length of questions because the user can keep adding cards if they want. Each card will have the same questions but they can type in unique answers. I have Trello which is kind of what I want but I want a front end to it where the user doesn’t have to think about trello settings, just typing in the boxes.
