Someone once told me: “The quality of any product is limited by the quality of the process creating it”. I’ve been obsessed with process improvement ever since. How do you improve your software production process?

I'm interested in the approaches of software process improvement for companies big and small. I'm aware with the traditional methods, like Retrospective meetings, but I'm not convinced these are enough.
