What is a good tool for users to report bugs and changes?

What tool(s) do you use to collect feedback on bugs or changes with your customers? I'm looking for something that would allow a user to mark up a screen or record screencasts that is available on the site if possible. Ideally this would work for both web and mobile apps and would integrate with project management software. Thanks.
